建國科技大學CTU表單系統個人資料蒐集、處理及利用告知聲明及同意書
本校依據個人資料保護法（以下簡稱「個資法」）第八條第一項規定於蒐集個人資料時明確告知下列事項，本校於下列事項範圍內，得蒐集、處理及利用您個人資料。
為保障您的權益及協助您瞭解本校如何蒐集及使用個人資訊，請務必詳細閱讀本聲明書及同意書之各項內容。
In accordance with Article 8, Paragraph 1 of the Personal Data Protection Act (hereinafter referred to as the "PDPA"), our school explicitly informs you of the following matters when collecting personal data.
Our school may collect, process, and utilize your personal data within the scope of the following matters.
To protect your rights and assist you in understanding how our school collects and uses personal information, please be sure to read this statement and consent form carefully.
一、蒐集之目的及用途 / Purpose and Use of Collection：
- 本人授權建國科技大學使用本人所提供的資料做為申請就讀華語課程、教務與 學務之用。
I authorize Chienkuo Technology University to use the information provided by me for admission application, teaching and academic affairs.
- 本人授權建國科技大學查驗本人所提供的所有入學申請資料。
I authorize Chienkuo Technology University to check all the admission materials provided by me.
- 建國科技大學華語中心取得您的個人資料將用於您的申請就讀華語課程與相關教務、學務相關單位工作之用。
Your personal information obtained by the Center of Chinese Language of Chienkuo Technology University will be used for your application for admission and the work of relevant educational and academic affairs units.
- 申請人所填各項資料，依據中華民國「個人資料保護法」等相關法令規定為必要之資訊應用處理相關事宜。
All the information filled by the applicant shall be processed for necessary information application in accordance with relevant laws and regulations such as the "Personal Data Protection Law" of the Republic of China.
二、蒐集之內容 / Content of Collection：
1.中文姓名Chinese Name
2.身分證字號(視業務需要) ID Number (as needed for business)
3.出生年月日(視業務需要) Date of Birth (as needed for business)
4.聯絡電話 Contact Phone Number
5.電子郵件信箱 Email Address
6.服務單位(視業務需要) Service Unit (as needed for business)
7.主辦單位因業務需求所需收集之資料，由主辦單位另訂之。Data collected by the organizing unit as required for business, as specified by the organizing unit.
三、利用之期間、地區、對象 / Duration, Area, and Subjects of Use：
(一)期間：自您填寫至本次業務結束後一年為止，期間由主辦單位保存您的個人資料，以作為本人、主辦單位查詢、確認證明之用。
Duration: Your personal data will be retained by the organizing unit for one year after you fill out the form, for the purpose of inquiries and verification by yourself and the organizing unit.
(二)地區：用於活動主辦單位提供服務之地區。
Area: The area where the organizing unit provides services for the event.
(三)對象：主辦單位，如因業務需要而交付個人資料予合作單位或廠商，將要求該單位或廠商於使用期間後刪除。
Subjects: The organizing unit. If personal data is provided to partners or vendors due to business needs, it will be required that those partners or vendors delete the data after the usage period.
四、依個資法第三條規定，資料填寫者對個人資料於保存期限內得行使當事人權利。
According to Article 3 of the PDPA, the data subject may exercise their rights over personal data within the retention period.
五、您可自由選擇是否提供資料，若其提供之資料不足或有誤時，將可能導致無法滿足此次業務作業
You are free to choose whether to provide data. If the provided data is insufficient or incorrect, it may prevent the completion of this business operation.
